Pediatric Health Choice (PHC) specializes in providing a comprehensive array of health care services to children with short-term, intermittent and/or complex chronic care needs.  Services are coordinated and provided in the home or in an ambulatory care center after discharge from the hospitals.

PHC is a subsidiary of Child Health Holdings, Inc., a Delaware Corporation.  We have specialized in pediatric care since 1991, are accredited by CHAP (Community Health Accreditation Program), certified by Medicaid and fully licensed by AHCA (Agency for Health Care Administration).

PHC provides a One Stop Care Coordination Model.  This model enables physicians, discharge planners and case managers to easily order all needed services upon discharge and to provide continuity of patient care. 

PHC is the preferred pediatric provider for home and community based care for many insurance companies, Medicaid, Children Medical Services (CMS) and other public funding programs.  We are the provider of choice for the care of children with complex medical needs.  

PHC currently operates in Florida and has the clinical capability to manage all levels of acuity of care at discharge.  We provide the following services in the child's home and/or at one of our Medical Day Health Centers:
