In Florida, Pediatric Health Choice is a licensed pediatric home health agency that provides a broad array of home health services to infants and children who require skilled nursing intervention. Within our family of companies, coordination of care often plays a vital role in achieving our overall mission. Therefore, in regions where we are not chartered to perform home health care directly we coordinate services with the child’s designated home health nursing provider, durable medical equipment vendor or pharmacy. This service delivery model promotes a coordinated medical support system assuring optimum care for the child.

As children’s clinical conditions improve and their medical needs change, a seamless transition may be made from home health care to one of our Medical Day Health Centers or into other community agencies. This uninterrupted, coordinated continuum of care, with its built-in support network, is invaluable to medically complex, technology-dependent children and their families.
